Provident Fund Savings Account

• Citizenship Certificate / National Identity Card / Passport / Driving License or other valid identification documents issued by the Government.

• Recent passport-size photograph.

• PAN, where applicable or required.

• Provident Fund Membership Certificate / Provident Fund Account Number / Membership Card issued by the concerned Provident Fund authority.

• Employment Certificate / Appointment Letter / Employee ID Card issued by the concerned organization, where applicable.

• Documents evidencing employment and/or contribution to the Provident Fund.

• Salary Certificate / Pay Slip or other documents evidencing source of income, where applicable.

• Address verification documents such as utility bill or other acceptable documents.

• Duly completed and signed Provident Fund Savings Account Opening Form.

• Nominee details and identification documents, where applicable.

• Specimen signature of the account holder.

• FATCA/CRS declaration and other applicable regulatory forms, where applicable.

• In case of change of address, copy of the relevant address verification document.

• Any other documents if felt necessary and required by the Bank.

Government Employee / Institutional Employee

• Citizenship Certificate / National Identity Card or other valid identification document.

• Recent passport-size photograph.

• Employee ID Card / Employment Certificate.

• Provident Fund Membership Certificate/Card or Provident Fund Account Number.

• Appointment Letter or other employment verification document, where applicable.

• Salary Certificate / Pay Slip, where applicable.

• PAN, where applicable or required.

• Address verification documents.

• Duly completed and signed Provident Fund Savings Account Opening Form.

• Nominee details and identification documents, where applicable.

• Any other documents if felt necessary and required by the Bank.

Provident Fund Account Through Authorized Representative

• Valid identification document of the account holder.

• Recent passport-size photograph of the account holder.

• Provident Fund Membership Certificate/Card or Provident Fund Account Number.

• Valid Power of Attorney / Authorization Letter, where applicable.

• Citizenship Certificate / Passport and recent passport-size photograph of the authorized representative.

• Documents evidencing employment and Provident Fund membership.

• Source of income/funds documents, where applicable.

• Address verification documents.

• FATCA/CRS declaration and other applicable regulatory forms, where applicable.

• Any other documents if felt necessary and required by the Bank.

General Requirements for Provident Fund Savings Account

• Duly completed and signed Provident Fund Savings Account Opening Form.

• Valid KYC documents of the account holder and authorized person, where applicable.

• Specimen signature of the account holder/authorized person.

• Valid proof of Provident Fund membership.

• Documents establishing employment/source of income, where applicable.

• Nominee details and identification documents, where applicable.

• Beneficial ownership details, where applicable.

• FATCA/CRS declaration, where applicable.

• Any required recommendation, approval or supporting document from the concerned Provident Fund authority/employer.

• Any other documents/information as may be deemed necessary and required by the Bank in accordance with applicable laws, regulations, NRB directives and internal policies.
